What if you have no proof of identity?

You can find some information on what is required in our Settlement.Org What documents do I need to apply for a Health (OHIP) Card? article.

In terms of documents that support your identity, you will find a listing of what the options are. For the purposes of the Ministry of Health and Long-Term Care, a document that supports your identity is a document that has both your name and your signature.
