To apply for Ontario Works, call or visit www.ontario.ca/socialassistance To receive held regarding utility assistance, individuals on Ontario Works (OW) or Ontario Disability Support Program (ODSP) can drop in with their utility bill and fill out a pink form (OW) or a green form (ODSP) * individuals who are not receiving social assistance can call the office for an appointment to fill out the Community Homelessness Prevention Initiative (CHPI) application to see if they qualify for assistance
