What if my Ontario birth certificate is lost or stolen?
If you were born in Ontario you can apply for a replacement birth certificate through Service Ontario.
To replace your birth certificate, you can:
You can apply for a birth certificate if you are:
- The person named on the certificate - that is, the certificate is for you. You must be 13 years old or older to apply.
- The parents of the person named on the certificate.
- The next of kin or executor, if the person named on the certificate has died.

If you were born in another province in Canada, you need to contact the ministry responsible for birth certificates in that province.
If you were born outside of Canada and have lost your birth certificate, you need to contact the consulate or embassy representative in Canada from your country of origin.
