What if my Health Card (OHIP) is lost or stolen?

Call the Ontario Ministry of Health INFOline:

• TTY: 1-800-387-5559

If you had a Health Card with a photo the Ministry will send you a new card.

If you had a red and white Health Card you will have to apply for a photo Health Card.

If you find your lost card, keep it only until your new card arrives and then destroy it because it will no longer be valid. Make sure you have your new photo Health Card before you destroy the old card.
